Hiperdia program patient care: a multiprofessional view
Abstract
Systemic arterial hypertension and diabetes mellitus constitute chronic noncommunicable diseases of high incidence in the Brazilian population. Objective: To report an experience from the multiprofessional monitoring of Hiperdia patient patients registered in a family health strategy in the municipality in southern Bahia. Method: Active search for patients with systemic arterial hypertension and/or diabetes mellitus in the territory was performed by the community health agent and then he was scheduled for care. On the day of the service participated in a multiprofessional group to promote the realization and guidance pertinent to pathology (s), after being attended to the shared care format between nurse and doctor. Results: From the monitoring of these users, the strengthening of their bonds with the family health strategy was observed; The exchange of professional knowledge in different fields within the same scenario, as well as the addition of the follow -up card. Conclusion: Thus, this work sought to report this new look at the Hiperdia program, in order to provide health care and care to this audience.
