Therefore, there was a need to reorganize the work process of the PHC teams to achieve the goals established by the program. Objective: To report the experience of a PRMSF/UESC resident nurse in an interventionist activity for the implementation of the Previne Brasil Program, indicators and goals to be achieved by the municipality in the financing of PHC. Methodology: This is a descriptive study with a qualitative approach through an Experience Report, experienced between August 10th and December 12th, 2022. Results and discussion: Of the 11 invitations sent to the teams, 83.33% (n=10 ) were filled. It resulted in the process of raising awareness about the theme and building a technical-scientific framework through the permanent education process associated with the principles of the National Policy on Humanization and meaningful learning. In addition to the reorganization of the work process, there was the opening of spaces for healthy dialogue, stimulation of autonomy, professional development and shared management among team members. Involving prevention, promotion, treatment, rehabilitation and palliative care at home, it is a possibility of care that goes beyond the physical structure of an institution. It is clear that there is a need to broaden the debate on this topic, since, in addition to presenting itself with more and more demands and being an area that requires humanized practices and with a psychosocial perspective from any health professional, it was a context strongly affected by the pandemic. of COVID-19. This is a qualitative work, in the form of an experience report. The central axis was the experience of carrying out home visits and the methodological course took place through the reading of notes in field diaries. From this, there was a phenomenological reduction, enabling a global and pre-reflective understanding. Then, the selection of themes that were considered of great importance in this experience was carried out, thus having an elucidation of the experience, interpretation and considerations. In this way, we sought to describe the experience and discuss about 5 units of meanings. It was possible to perceive how much this possibility of assistance has varied demands, whether individual, family or community, and the importance of returning government incentives to strengthen team health practices. This study has the general objective of understanding how territorialization in health has been used as an instrument for the practice of psychology in Primary Health Care and as specific objectives (a) to identify the objectives of the studies (b) to identify the types of studies ( c) analyze the intervention strategies developed/presented. It is a bibliographical research, of the integrative review type, with a qualitative approach. Scientific Electronic Library Online (SciELO), Latin American and Caribbean Literature in Health Sciences (LILACS) and Google Scholar were used as databases to perform searches, prepare a review and integrate the data found. Most studies bring the conception of the living territory, pointing out that only by making a precise analysis of the territory is it possible to raise more assertive discussions to handle the cases, therefore discussing the importance of the territory and its recognition beyond the geographic space , as well as mental health care in this context. Despite this, there is a lack of articulation of the importance of this territory and of territorialization as a potentiating instrument for practices and intervention planning by psychologists in PHC. Objective: To report an experience from the multiprofessional monitoring of Hiperdia patient patients registered in a family health strategy in the municipality in southern Bahia. Method: Active search for patients with systemic arterial hypertension and/or diabetes mellitus in the territory was performed by the community health agent and then he was scheduled for care. On the day of the service participated in a multiprofessional group to promote the realization and guidance pertinent to pathology (s), after being attended to the shared care format between nurse and doctor. Results: From the monitoring of these users, the strengthening of their bonds with the family health strategy was observed; The exchange of professional knowledge in different fields within the same scenario, as well as the addition of the follow -up card. Considering that intersectoral partnerships are powerful mediators and strengthening, this study aimed to report the performance of multiprofessional resident professionals in Family Health in the construction of intersectoral partnerships with the Reference Center for Social Assistance. The study is an experience report, in descriptive modality, in which the lived experiences were extracted from the Reflective Portfolio between the years 2021 and 2022. Meetings were held between teams and health promotion activities for the assisted, with the main methodology being the conversation wheels. Therefore, it is verified that the construction of intersectoral partnerships stimulated teamwork, communication and promoted interprofessional collaborative practice; ensured the joint participation of the health and social assistance sectors in commemorative events, awareness campaigns and health education activities; allowed the realization of individual visits in the spaces; ensured comfort, welcoming, privacy and comprehensive care to individuals by stimulating self-care, autonomy, co-responsibility and empowerment. This requires the finding that, despite the difficulties faced, such as the predominance of the biomedical model in the Family Health Unit and the focus on the Bolsa Família Program, resident professionals were able to build the intersectoral partnership between the Family Health Unit and the Social Assistance Reference Center.</p>2024-12-05T15:33:02-03:00##submission.copyrightStatement##
